    We have provided you with a printable PDF of our new patient paperwork, both obstetrics and gynecology, to be filled out prior to your visit with the doctor.

    Please fill these forms out in black ink only.

    We kindly ask that when you arrive for your scheduled appointment that you give the completed paperwork to the receptionist to be entered in our system.

    It is also very important that you bring with you for your first appointment the items listed below:

    • Insurance Card(s)
    • Primary Form of ID: Current Drivers license or Picture ID card
